What is a Non-Conformance Report (NCR)?
In aerospace, defense, and medical manufacturing, a Non-Conformance Report (NCR) is a formal document initiated whenever a physical workpiece, raw material lot, assembly, or test procedure fails to meet specified engineering drawings, purchase order specifications, or quality standards.
Under AS9100 Clause 8.7 (Control of Nonconforming Outputs), organizations must maintain strict procedures to identify, segregate, evaluate, and dispose of non-conforming items to prevent unauthorized shipment or unintended installation into flight assemblies.
The 5 Stages of the NCR Lifecycle
1. Identification & Segregation
The moment an inspector or machinist discovers an out-of-tolerance dimension (e.g., recorded on AS9102 Form 3 Field 11):
- The part is immediately tagged with a physical red rejection tag.
- The material is removed from the CNC work cell and placed into a locked Quarantine / MRB Cage.
- The active job traveler is paused to prevent downstream machining operations.
2. Material Review Board (MRB) Disposition
A multi-disciplinary team consisting of Quality Engineering, Manufacturing Engineering, and Customer Quality Representatives evaluates the defect and issues an official disposition:
| Disposition Type | Operational Meaning | Customer Approval Required? |
|---|---|---|
| Scrap | Part cannot be salvaged or economically reworked. Must be physically mutilated and discarded. | Internal quality approval only (unless customer-furnished material). |
| Rework | Part can be brought into 100% compliance with original drawing tolerances (e.g., skim-cutting an oversized diameter). | Generally no customer concession required if fully brought to print. |
| Repair | Part is modified using an approved engineering repair procedure (e.g., bushing an oversized hole, weld build-up). | Yes — Customer Concession / Waiver strictly required in aerospace. |
| Use As-Is (UAI) | Engineering analysis confirms the deviation does not affect fit, form, function, safety, or fatigue life. | Yes — Formal Customer Deviation Request required before release. |
| Return to Vendor (RTV) | Defect originated from raw material supplier or subcontracted special processor. | Vendor notification and commercial debit memo. |
The 8D Root Cause Problem-Solving Methodology
When high-severity or repeat non-conformances occur, customers mandate a formal Eight Disciplines (8D) corrective action investigation to prevent recurrence:
- D1Establish the Team: Assemble cross-functional personnel with process knowledge (quality engineer, machinist, programmer, tooling specialist).
- D2Describe the Problem: Define the issue using the 5W2H method (What, Where, When, Who, Why, How Many, How Often) referencing exact drawing balloon numbers.
- D3Implement Interim Containment Actions (ICA): Isolate inventory in transit, work in progress (WIP), and finished goods stock to protect the customer from receiving defects.
- D4Identify & Verify Root Cause: Conduct Ishikawa (Fishbone) diagramming and 5-Why analysis to determine why the defect was created and why it was not detected earlier in the workflow.
- D5Choose Permanent Corrective Actions (PCA): Design engineering solutions (e.g., updating CNC tool offsets, installing poke-yoke error-proofing fixtures, modifying CAM toolpaths).
- D6Implement & Validate PCAs: Execute the fix on production equipment and run validation batches with capability analysis ($C_{pk} \ge 1.33$).
- D7Prevent Recurrence: Update Standard Operating Procedures (SOP), work instructions, failure mode effects analysis (FMEA), and internal audit schedules.
- D8Congratulate the Team: Formally close out the CAPA with customer sign-off and document lessons learned.
Traceability: Linking Balloon Numbers to NCRs
The foundation of non-conformance management is characteristic-level traceability. An NCR stating "the hole is oversized" is vague and gets rejected by aerospace auditors. An NCR stating "Characteristic #27 (Sheet 1, Zone D3) measured ⌀ 12.75 mm vs specification 12.70 ± 0.02 mm" provides instant, unambiguous traceability.
